Reduce your waiting time by completing our new patient form before you arrive. Either fill in the online form or print out and bring along to your appointment.  

Please arrive 5 minutes early to your first appointment and remember to bring your Medicare card. If you do not have a Medicare card, please bring photograph identification.

New Patient FAQs

Do I need to register as a new patient?

Yes. Please fill in the online form or print out the PDF and either bring it to your appointment or email to our reception. Alternatively, you can collect one from our reception, but we would ask you to arrive 5-10 minutes early to allow time to complete the form.

What should I bring to my first appointment?

You will need to provide your Medicare Card or photographic identification (e.g. passport or an Australian driver’s licence, as well as a list of your medications and any significant past medical history.

What is the cost for my consultation?

We are a private billing practice meaning we charge consultation fees, depending on the length of your appointment.

Extended Consultation $110
Comprehensive Consultation $145

Children under 16, pension and health care card holders are bulk billed

Accounts must be paid at the end of a consultation. Our reception staff can assist you to claim your Medicare rebate.

We honour current Health Care and Pension Cards Monday through Friday.

On average the out of pocket expense is less than $40. Please feel free to call our reception if you have any questions or would like an estimate of our fees.

Do you send SMS reminders prior to my appointment?

Yes. Providing we have your current mobile number on record, you should receive a reminder text prior to your appointment.

How do I cancel my appointment?

You can cancel your appointment by ringing our reception on 9443 2507. Please be aware that we do require a minimum of 3 hours notice to avoid cancellation fees.

How do I find out about my test results?

Our practitioners review a large number of results every day, therefore if your results are within the normal range you will not be contacted.

If your test results come back as abnormal we will contact you via phone or email.
