New Patient FAQs
Do I need to register as a new patient?
Yes. Please fill in the online form or print out the PDF and either bring it to your appointment or email to our reception. Alternatively, you can collect one from our reception, but we would ask you to arrive 5-10 minutes early to allow time to complete the form.
What should I bring to my first appointment?
You will need to provide your Medicare Card or photographic identification (e.g. passport or an Australian driver’s licence), as well as a list of your medications and any significant past medical history.
What is the cost for my consultation?

Do you send SMS reminders prior to my appointment?
Yes. Providing we have your consent and current mobile number, you should receive a reminder text prior to your appointment.
How do I cancel my appointment?
You can cancel your appointment by ringing our reception on 08 9443 2507. Please be aware that we do require notice of a minimum of 3 hours to avoid cancellation fees.
How do I find out about my test results?
Our practitioners review a large number of results every day, therefore if your results are within the normal range, you will not be contacted.
