Diabetes

Putting you at the centre of your care team

Insulin is the hormone that enables our body cells to use glucose. A person with type 1 diabetes needs daily insulin injections to stay alive, as their pancreas cannot make insulin. A person with type 2 diabetes cannot make or use insulin properly.

Without regular checks, diabetes can lead to complications that can affect the whole body, including the kidneys, eyes, feet, nerves and heart.

We use an annual cycle of care to help you manage your diabetes, and to reduce your risk of diabetes-related complications.

The annual cycle of care includes the following health checks:

  • HbA1c
  • Blood pressure
  • Foot assessment
  • Eye examination
  • Kidney health
  • Blood fats (cholesterol and triglycerides)
  • Weight, waist and body mass index
  • Healthy eating
  • Physical activity
  • Medication review
  • Smoking
  • Diabetes management
  • Emotional health
  • GP management plan
