Chronic Disease Management
A chronic condition is one that lasts a long time and doesn’t go away. If not managed well it will likely get worse over time and cause increased dependence on others.
Chronic conditions can be managed well if you follow a healthy lifestyle and follow suggested treatments according to symptoms.
Chronic conditions may include:
- Asthma
- Arthritis
- Diabetes
- Coronary Heart Disease
- Osteoporosis
- COPD
- Mental health such as depression and anxiety
- Hypertension
The goal to manage chronic disease is to first identify persons at risk, to promote self-management and improve (or at least maintain) quality of life.
